In an emergency, dial 911 first, then notify Campus Safety.
Safety concerns and crimes
Criminal offenses, emergencies, and safety concerns are reported to the Department of Campus Safety, Room A-108B — ext. 2911 from any campus phone; (330) 555-0140 ext. 2911 from off campus — or to any College employee. Reports may be made anonymously by telephone, by mail, or by written note delivered to Room A-108B. Anonymous reports are recorded in the daily crime log and are included in Clery statistics; the College notes that anonymous reports limit its ability to investigate or to provide supportive measures to a complainant. Sexual harassment, sexual assault, dating violence, domestic violence, and stalking
Reports are made to the Title IX Coordinator, Bernadette Ollsvig, Room A-118, ext. 2240, or to Campus Safety, or to law enforcement, or to any combination of these, or to none. Supportive measures are available without a formal complaint, and a complainant is not required to report to law enforcement in order to receive them. Confidential support is available through the Tolliver County Victim Assistance Program, (330) 555-0177, which is independent of the College. Procedures are described at Title IX.
Suspected abuse or neglect of a minor
All College employees report suspected child abuse or neglect to Tolliver County Children Services and to law enforcement, directly and immediately. Reporting is not routed through a supervisor or any College office first.
Academic concerns
A concern about a grade, a course, or instruction begins with the instructor of record. A concern not resolved with the instructor goes to the division chair, and then to the Office of the Provost, Room A-202, ext. 2205. Grade appeal deadlines and procedures are published in the College Catalog.
Student conduct
Concerns regarding student conduct, and concerns regarding a student’s welfare, are reported to the Office of the Dean of Students, Room A-130, ext. 2140.
Fraud, waste, and financial concerns
The Auditor of State maintains a system for reporting fraud, including misuse of public money, by any public office or public official. Reports may be made anonymously to the Auditor of State’s fraud line at (866) 372-8364, by email to fraudohio@ohioauditor.gov, or through the reporting form at ohioauditor.gov. Concerns may also be reported to the Vice President for Finance and Administration through the Office of the President, Room A-200, ext. 2201. No College employee may retaliate against a person who makes a report in good faith.
Maintenance requests
Facilities conditions — leaks, lighting, heating and cooling, pavement, and similar matters — are reported to the Facilities Department, ext. 2120. A condition presenting an immediate hazard is reported to Campus Safety at ext. 2911 at any staffed hour.
